What on Earth Is a DUTCH Test, and Do I Need One?

DUTCH stands for Dried Urine Test for Comprehensive Hormones. It is, in my view, the single most useful hormone test available to women today, and it is also the one most likely to be ordered when it does not need to be.

First, what it is. Over the course of a single day you collect four small dried urine samples on a piece of filter paper. You send them in a small envelope to a lab. About three weeks later you receive a beautiful, detailed report showing not only your sex hormones, but how your body is metabolising them.

That last part is the magic. A standard blood test for oestrogen tells you how much oestrogen is sitting in your blood at the moment of the draw. The DUTCH tells you whether your body is breaking that oestrogen down through the protective pathway, the neutral pathway, or the pathway that has been associated with greater breast cancer risk. It tells you whether your liver is methylating well. It tells you whether your gut is recycling oestrogen back into the body. It tells you your cortisol pattern across the whole day, not just at the moment you had your blood drawn. It tells you whether your stress system is overactive, underactive, or burnt out.

In perimenopause, in PMOS, in stubborn hormonal acne, in unexplained PMS, in oestrogen-dominant patterns, in women coming off the pill and trying to make sense of what is left, the DUTCH is genuinely useful. It changes protocols. It saves time. It catches the missing piece.

Who should not bother? Women in the middle of a perfectly regular cycle with no symptoms and no concerns do not need it. Young women whose foundations have not yet been addressed - sleep, nutrition, stress, blood sugar - do not need it. You cannot test your way out of a lifestyle question. You can only test your way into a more precise answer once the foundations are in place.

If we work together and a DUTCH would meaningfully change the path forward, I will tell you. If it would not, I will save you the money. That is how I work, with every test, every time.
